私の家族には、最重度知的障害・難治性てんかん・自閉症をもつ9歳の双子の娘がいます。
この記事では、東京都内で障害児を育てる家庭が利用できる支援制度について、私が実際に受けている手当やサービスをもとにまとめました。
2025年8月時点の最新情報ですが、制度や金額は今後変更される可能性があります。申請前には必ずお住まいの市区町村役所でご確認ください。
東京都で受けられる全国制度
特別児童扶養手当（全国制度／東京都内で申請）
- 支給額（2025年4月改定後）
1級：月額 56,800円
2級：月額 37,830円 - 対象：政令で定める程度の障害のある20歳未満の児童を養育している父母または養育者
- 所得制限：扶養人数に応じてあり
- 申請先：市区町村役所（例：障害福祉課）
わが家の場合
初回申請時に医師の診断書に追加記載が必要と言われ、再度書いていただきました。申請から支給開始までの期間は自治体や状況によって異なるため、早めの準備をおすすめします。
障害児福祉手当（全国制度）
- 支給額（2025年4月改定後）：月額 16,100円
- 支給時期：年4回（2月・5月・8月・11月）
- 所得制限：あり
- 対象：重度の障害があり、常時介護を必要とする20歳未満の児童
わが家の場合
各手当で別々の診断書や申請書が必要でした。同時期に申請したため通院や役所への往復をまとめられましたが、それでも手間はかかります。
東京都独自・自治体の制度
心身障害者医療費助成制度（マル障）
- 対象：身体障害者手帳1〜2級、愛の手帳1〜2度、精神障害者保健福祉手帳1級など
- 所得制限：あり
- 内容：医療費の自己負担分（外来・入院）の助成
東京都例（2025年8月時点）
外来：月額上限 18,000円
入院：月額上限 57,600円
※食事療養費や差額ベッド代は対象外
※わが家の場合、脳梁離断の手術で差額ベッド代が発生しました。後日、体験記事として詳しく書く予定です。
わが家の場合
救急搬送や入院が多いため、この制度は本当に助かっています。医療費の高額負担が大幅に減ることで、精神的な安心感も得られました。
紙おむつ支給事業（自治体独自）
- 多くの自治体で実施（対象年齢や支給枚数は異なる）
- 例：品川区では毎月宅配で支給、他の区は年数回まとめて支給
わが家の場合
毎月、役所から宅配で届くため買い物の手間や持ち運び負担がなく、とても助かっています。
乳幼児発達支援利用者負担第1子無償化（東京都独自）
- 開始：2025年9月1日
- 内容：0〜2歳の第1子利用者負担を、所得に関係なく無償化
- 対象事業：児童発達支援、医療型児童発達支援、居宅訪問型児童発達支援

- 市区町村役所：障害福祉課や子ども家庭支援センター
- 東京都福祉保健局 障害者施策推進部
- 東京都心身障害者福祉センター（判定・相談機関）
利用して感じたこと
制度は「申請しないと始まらない」ものです。
書類や診断書の準備は大変ですが、受給が始まると家計面・精神面で大きな支えになります。
ぜひ遠慮せず、利用できる制度は活用してほしいと思います。

My family has 9-year-old twin daughters with severe intellectual disabilities, intractable epilepsy, and autism.
This article summarizes the support systems available to families raising children with disabilities in Tokyo, based on the allowances and services we actually receive.
As of August 2025, the latest information is provided here. Please note that systems and amounts may change in the future. Before applying, always confirm with your municipal office.
National Programs Available in Tokyo
Special Child Rearing Allowance (National Program / Apply in Tokyo)
- Payment amount (after April 2025 revision):
Grade 1: ¥56,800/month
Grade 2: ¥37,830/month - Eligibility: Parents or guardians raising children under 20 years old with disabilities defined by government ordinance
- Income limit: Varies based on number of dependents
- Application: Municipal office (e.g., Department of Disability Welfare)
In our case
At the first application, the doctor’s certificate required additional notes, so we had it rewritten. The period from application to start of payment varies depending on the municipality and situation, so early preparation is recommended.
Tokyo Metropolitan Government: Special Child Rearing Allowance
Welfare Allowance for Children with Disabilities (National Program)
- Payment amount (after April 2025 revision): ¥16,100/month
- Payment frequency: 4 times/year (February, May, August, November)
- Income limit: Yes
- Eligibility: Children under 20 years old with severe disabilities requiring constant care
In our case
Separate medical certificates and application forms were needed for each allowance. Since we applied during the same period, we were able to combine hospital visits and municipal trips, but it still required effort.

Tokyo-Specific and Municipal Programs
Medical Expense Subsidy for the Disabled (Marusho)
- Eligibility: Physical Disability Certificate grades 1–2, Ai-no-Techo grades 1–2, Mental Disability Certificate grade 1, etc.
- Income limit: Yes
- Coverage: Copayments for outpatient and inpatient medical costs
Tokyo example (as of August 2025)
Outpatient: Max ¥18,000/month
Inpatient: Max ¥57,600/month
Food therapy costs and extra bed charges are not covered.
In our case, extra bed charges occurred during a corpus callosotomy surgery. I plan to write a detailed article about this later.

Because we often face emergency transport and hospital admissions, this system has been truly helpful. It greatly reduces the heavy financial burden of medical expenses, bringing a sense of relief.
Tokyo Metropolitan Government: Medical Expense Subsidy
Diaper Supply Program (Municipal)
- Implemented in many municipalities (age limits and supply amounts vary)
- Example: In Shinagawa Ward, monthly home delivery; in other wards, delivery several times a year in bulk
In our case
Monthly home delivery from the municipal office eliminates the hassle of shopping and carrying heavy packages, which is a huge help.
Free First-Child User Fees for Infant Development Support (Tokyo-Specific)
- Start date: September 1, 2025
- Content: Waives user fees for first children aged 0–2, regardless of income
- Target programs: Child development support, medical-type child development support, home-visit child development support
